Who is home recovery care for

If your condition can be safely treated outside the hospital, you may qualify for home recovery care. Examples include:

  • Medical care (around 30 days): Supports inpatient and shorter observation episodes when appropriate, can include pneumonia, congestive heart failure and, urinary tract infections
  • Rehabilitation care (around 60 days): Infusions, wound care and physical or occupational therapy

Your recovery care coordinator will work with you and your doctor to confirm eligibility and the right program.

How it works

  • Go home instead of being admitted (when medically appropriate)
  • We set you up with any needed equipment, including a tablet. at no additional cost (returned at the end of care)
  • Meet your recovery care coordinator, your single point of contact who:
    • Coordinates with your doctor(s)
    • Monitors daily vitals
    • Answers medication questions
    • Assists with follow-up appointments
    • Provides health education
  • Your nurse team cares for you at home typically 3–5 days for medical care (longer if needed) and 2–3 weeks for rehab care
  • Virtual visits: where you see and talk to your doctor from home on the tablet
