What’s changing?
We are changing our patient portal. We are moving from Syntellis to a new portal called FollowMyHealth.
When is this happening?
- The Syntellis patient portal will permanently close on April 30, 2026.

After April 30, information in Syntellis will no longer be available and cannot be recovered.

Please save anything you want to keep before April 30, 2026.

All your medical health history will still be available in the
electronic health
record.
What do you need to do now?
Log into Syntellis before
April 30, 2026
and download or save:
- Test results
- Visit summaries
- Documents you want to keep
Also, you do
not need to sign up for the new portal yet. You will receive an invitation.
How to download information from Syntellis?
You can download your information directly from the Syntellis portal. While screens may look a little different, these steps usually work:
Log into the
Syntellis patient portal
Go to sections such as Health Records, Test Results or Documents
Use available Download, Export or Save options
Save files to a personal computer, phone or secure cloud storage
You may need to repeat this process for different sections of the portal.
What is FollowMyHealth?
FollowMyHealth is a
secure online patient portal. It lets you:
How will I access FollowMyHealth?
You do not need to sign up on your own. You will receive an automatic invitation to FollowMyHealth after your next visit, with instructions on how to create your account.
Will my old information move to FollowMyHealth?
Some information will appear in FollowMyHealth. However, older information that exists only in Syntellis may not appear. You can save for your records by downloading before April 30, 2026.
Will all my medical health records be gone?
No. You will still have all your medical history on our electronic health record. If you need to request records, you can
submit a request online or contact our Health Information Management team at 1-800-782-8581, option 3 or by email
medicalrecords-mfld@sanfordhealth.org.
