The Marshfield Clinic-Saint Joseph's Hospital Post-Doctoral Psychology Fellowship Application is available here to print and complete. Along with the completed application, you must send three letters of recommendation (with particular reference to your clinical skills) and an official graduate transcript. Please ask your references to include their email address in their letter. Note, your submitted application is also your consent for us to contact your graduate program, internship program, and references. Application, letters of recommendation, and transcripts should be sent electronically (or faxed) and mailed to the address listed below.

The Marshfield Clinic Post-Doctoral Psychology Fellowship Program will begin accepting applications for its two positions (Child/Adolescent Fellowship and Pediatric Fellowship) beginning in the fall of 2010. Both of these fellowships will start in September of 2011.

Additional requirements:

The post-doctoral psychology fellow is expected to have completed all requirements for a doctoral degree in psychology. The fellow must have in his/her hand on the first day of the fellowship a diploma or letter from the director of graduate studies verifying the completion of all degree requirements pending institution graduation ceremony.

The post-doctoral fellow must have completed an internship meeting APPIC standards.

At the beginning of the post-doctoral psychology fellowship, the fellow must have 3,000 hours of supervised experience in clinical practice. Per the guidelines specified by WI Code for Outpatient Psychotherapy Clinics. Please review these guidelines at http://dhfs.wisconsin.gov/forms1/OQA/oqa2569.doc. Please note that internship and graduate practica hours may be included, but pre-master's degree hours cannot be included. Prior to final acceptance into the program you will need to obtain signed supervisor affidavits to confirm you have met the 3,000 hour requirement. Affidavits can be found at http://dhfs.wisconsin.gov/forms1/OQA/oqa2570.doc. We must have confirmation from the state of Wisconsin that they have received your application for individual provider status and the affadavits before you can start fellowship.

Applicants for the fellowship must (a) be licensed as a psychologist in the State of Wisconsin or (b) become eligible during the fellowship and agree to attain licensure as soon as possible during the fellowship. Obtaining your first license is a major milestone and one that involves considerable paperwork. If you accept a position with us you will be required to complete some parts of the licensure process the summer before you start fellowship. The supervision forms to verify internship hours MUST be completed by your internship supervisors and you MUST have them with you when you report for your first day. We will work with you during the summer to ensure these critical documents are completed so that you can start fellowship. Please review these forms at http://drl.wi.gov/dept/forms/fm2555.pdf.

Applicants for the fellowship must (a) be licensed as a psychologist in the State of Wisconsin or (b) become eligible during the fellowship and agree to attain licensure as soon as possible during the fellowship. The major requirements for license in Wisconsin include:

  • A doctoral degree in psychology.
  • Applicants for licensure must pass the national examination in the professional practice of psychology.
  • Applicants must pass an examination on relevant state laws and the ethical practice of psychology.
  • A personal interview may be required with the Psychology Examining Board of the State of Wisconsin.
  • Three thousand hours of experience supervised by a psychologist:
    • The first 1,500 hours are generally met by internship training.
    • The second 1,500 hours must be post-doctoral and can be accomplished during the post-doctoral psychology fellowship. Supervision provided during the fellowship year satisfies State of Wisconsin post-doctoral supervision requirements for licensure (Wisconsin State Psy2.09(3)).
